With over $13.2 million in penalties levied against Wisconsin nursing homes, navigating the labyrinth of regulatory inspection records has become a critical challenge for families. To bridge this transparency gap, Milwaukee-based PKSD Accident & Injury Lawyers launched a research tool aggregating federal and state data on facility safety and violation histories.

The digital initiative synthesizes records from CMS, NursingHomeDatabase.com, and ProPublica’s Nursing Home Inspect to present a consolidated view of 323 facilities across the state. By centralizing data on staffing levels, medication errors, infection-control failures, and abuse reports, the firm aims to provide families with a clear window into facility performance before placement decisions are finalized.

Wisconsin records reveal a troubling landscape: 132 homes currently show serious deficiencies, while 96 facilities have faced payment suspensions. Some institutions have logged extensive records of negligence, including one Milwaukee facility burdened by over $387,000 in fines and 115 individual complaints. According to Jeff Pitman of PKSD, the goal is to transform dense, government-mandated reports into actionable insights. The firm plans to expand the project with additional city-specific summaries and educational resources to further assist families in evaluating elder care standards.
