By deploying teams consisting of physicians, nurses, and social workers, the partnership aims to reduce hospital readmissions and provide consistent, relationship-based support for those managing serious illness.
"After nearly two decades caring for patients with serious illness in the greater Boston area, I've seen firsthand what consistent, community-based palliative care can do for patients and their families," said Mihir Kamdar, Chief Medical Officer of Tuesday Health. The program reflects a broader shift among health plans toward provider-led models that prioritize home-based clinical intervention over traditional facility-based care. For CCA, the move aligns with their mission to address the complex social and medical needs of their members, while Tuesday Health leverages its expertise to manage the logistical and emotional challenges families face during end-of-life care.
